MEMORY CARE
Memory Care is specialized care for people living with Alzheimer's and other forms of Dementia. Transition Assisted Living aims to give exceptional care to ALL of our Residents, but especially those in our memory care program who might require a bit more attention. Transition Assisted Living trains our Resident Attendant's (RA's) on basic facility safety protocol and security related issues as a baseline education, but we pride ourselves in our desire to focus on each Resident's unique personality and set of needs which may be ever-changing.
Creating a “fun living environment" is a primary goal of our community so that every Resident feels Safe, Satisfied, and Stimulated. Please always feel free to contribute ideas and ways in which we can create a “customized living experience" suited to your loved one's unique needs.
Alzheimer's Program Statement:
Transition Assisted Living LLC. is determined to provide a therapeutic and caring environment for residents with Alzheimer's disease and related conditions.
Prior to admission to Transition Assisted Living, each prospective resident with the support of their authorized representative (if necessary) will undergo a rigorous evaluation procedure. This evaluation will include, but is not lim/20ited to, assessing physical, emotional, mental health, and memory care needs of a prospective resident. Re-evaluation will be done on an annual basis, or more frequently as needed. If a time comes in which Transition Assisted Living is no longer the best suited home for the resident, Transition Assisted Living will support the resident and/ or responsible party in finding a new permanent residence.
All Resident Care Attendants will be trained in caring for residents with Alzheimer's and related conditions. All Resident Care Attendants will have continuing education provided on an annual basis and more frequently if required based on the resident's progression through the disease.
Transition Assisted Living provides an enclosed outdoor patio area to provide a safe, outdoor space for residents with Alzheimer's and related conditions. All exits are internally alarmed to notify the staff when a door is opened to assist with keeping the facility secure. The facility has security cameras in all public areas, as well as around the exterior of the building.
Transition Assisted Living LLC provides a daily activity program, which will incorporate activities specifically designed for residents with memory care needs on an as-needed basis. These activities include but are not limited to pet therapy, art therapy, and music therapy. The additional fees for memory care is an additional $1,550 per month, but may be higher or lower depending on the service plan of the individual resident and their needs.
Memory Care Includes:

- Tracking and/or attending of Resident's Doctor Appointments and aid them in getting ready for such appointments prior to family bringing them offside or practitioner arrival to an in-home appointment.

- Director Regular Updates to Resident Family Members with scheduled messages/updates with photographs, activities performed, residents general mood, eating habits, struggles, and concerns *Email or Technology App may be available at a future date

- Daily Meal Reminders and, if required, making sure they walk themselves to the dining room for meals

- Encouraging outdoor walks with a Resident Attendant one-on-one or in groups

- Daily Wardrobe Changing Reminders and Selection of Appropriate Attire for Dining, Bedtime, etc.

- Daily Reminders for general hygiene including, combing or brushing hair, brushing teeth, washing face *If additional hygiene services are required by staff beyond reminders are required, a personal care package would be required.

- Walking Resident to Showers for Regular Bathing (2 x's per week) *If additional showering or hygiene beyond reminders is required a personal care package would be required.

- Regular tidying of Resident's Room (above our normal housekeeping service) if required (taking out the trash, making the bed, putting away laundered items, general organizational tasks)

- Bedtime Turn-Down Service if desired by the Resident (turn on reading light, dim room lighting, close blinds, turn-down the sheets, prepare PJ's, and prepare a glass of water for bedtime).

- One-on-one attention from Staff with daily conversations as desired by Resident

- Encouraging and/or doing Daily Activities such as Puzzles, Crossword Puzzles, Board Games

- Encouraging and/or doing Group Activities with our Staff and Activities Providers to stay active *Group posted activity calendar available after 10 residents

- Encouraging regular phone calls and visits with Resident family members to communicate with Resident on a regular basis and to attend Resident in-home appointments via phone or in person

- Mind stimulating games like Trivia, Sudoku, Puzzles, Game Shows, and Crossword Puzzles are highly encouraged by our staff to keep the Resident's mind sharp!
